Founded in January 2026 and headquartered in Charlotte, Pinnacle Care PLLC is designed to improve clinical outcomes and care coordination for elderly and medically complex patients residing in skilled nursing and long-term care facilities. The practice emphasizes physician leadership, multidisciplinary collaboration, and evidence-based geriatric care.
Dr. Patel brings extensive expertise in acute inpatient medicine, ICU co-management, ventilator familiarity, rapid response leadership, and complex care coordination. Throughout his career, he has served hospitals and healthcare systems across multiple states through both full-time hospitalist practice and locum tenens assignments.

Prior to founding Pinnacle Care, Dr. Patel served as Medical Director for Summit Medical Carolinas from 2024 to 2025, where he oversaw physician and advanced practice provider services in skilled nursing and long-term care facilities. In that role, he led quality improvement initiatives, ensured CMS regulatory compliance, and directed hospital-to-SNF transition protocols and interdisciplinary care planning.
Earlier in his career, Dr. Patel spent more than a decade as a Hospitalist with Carolinas Hospitalist Group at CMC Union Hospital, where he managed adult inpatient care, participated in rapid response and code teams, and co-managed critically ill patients in the intensive care unit. He also served as Associate Medical Director, contributing to hospital quality initiatives and physician coordination efforts.
In addition to leading Pinnacle Care, Dr. Patel currently provides hospitalist coverage for rural and community hospitals through Rural Physicians Group, delivering inpatient admissions, rounding, cross-coverage, and discharge management while collaborating with multidisciplinary teams in diverse hospital environments.
Pinnacle Care PLLC is actively building partnerships with skilled nursing facilities throughout North Carolina while developing clinical infrastructure that includes integrated electronic medical records, billing and revenue cycle management systems, compliance frameworks, and coordinated care programs designed to support high-quality geriatric care.
Dr. Patel holds an MD from the Family Medicine Residency Program at the University of Alabama in Huntsville, a Master of Public Health, and an MBBS from Kasturba Medical College in India. He is board-certified by the American Board of Family Medicine and holds active medical licenses in North Carolina, South Carolina, and Colorado.
He is also a member of the American Academy of Family Physicians, the North Carolina Academy of Family Physicians, and the American Association of Physicians of Indian Origin.
